Grievance & Complaint Procedure (Draft)
Who this is for
Any student of Northwest FastPath CNA Training who has a concern about instruction, grading, a skills check-off, clinical placement, billing, or the conduct of staff or another student may use this procedure. Using it will never affect your standing in the program.
Step 1 — Raise it with your instructor
Most concerns are resolved fastest by talking to your instructor directly. Raise it as soon as you can, while the details are fresh.
Step 2 — Put it in writing to the program
If the concern isn't resolved, or you'd rather not raise it with your instructor, submit it in writing to tara@northwestfastpathcna.com or by phone at 763-227-5378. Include what happened, when, who was involved, and the outcome you're asking for.
The program will acknowledge your complaint in writing, review it, and give you a written decision. Your complaint, the review, and the decision are kept on file.
Step 3 — Contact the Minnesota Department of Health
You may contact the Minnesota Department of Health nurse aide program at any point — you do not have to complete Steps 1 and 2 first, and you do not need the program's permission. MDH approves and oversees nurse aide training programs in Minnesota: MDH nurse aide program.
Discrimination and harassment complaints
Complaints of discrimination, harassment, or a request for a disability accommodation are handled under the program's Nondiscrimination, Anti-Harassment, and Accommodation Policy, which has its own process. See also our Non-Discrimination Statement.
Retaliation
Northwest FastPath CNA Training does not retaliate against a student for raising a concern in good faith, or for taking a concern to MDH.
